Madalyn “Maddie” Clare Lively, 4 months old, passed away on Thursday, February 3, 2005 at her residence. Services will be held at 2:00 on Monday, February 7, in the chapel of McCullough Funeral Home with interment following in Parkway Memorial Gardens. Visitation will be Monday from 1:00 until 2:00 p.m. at McCullough Funeral Home.
Survivors include her parents, William Edward Taylor Lively and Karen Elizabeth Lively; sister, Kassandra Elizabeth Lively, twin sister, Katelyn Alane Lively, all of Warner Robins; paternal grandparents, Dale and Pam Lively, Warner Robins; maternal grandparents, David and Kimberly Foell, Kathleen; paternal great-grandparents, Melba and Everett Taylor, Jacksonville, FL; maternal great-grandparents, Darrell and Shirley Foell, Peever, SD.
